CHANGELOG: repoint commit links after history moved.

A rebase rewrote the commits these entries link to, so the links named commits
that no longer exist and would have 404ed on GitHub. Repointed by
releases/fix-changelog-hashes.sh, which only touches a link no ref in this clone
can reach.
